 | Every client is required to sign and fill-out five documents (which are included in the New Client Packet below), prior to the first session:
- New Client Form (in PDF and Word formats)
- Emergency Contact Release of Information (in PDF and Word formats)
- Family Background Information Form (in PDF and Word formats)
- Counseling Services Informational Contract (in PDF and Word formats)
- Professional Disclosure Statement (in PDF and Word formats)
All other documents are optional, but are helpful for the therapy process. |
